Donation fee for all clinics: $12.00 Administration fee per vaccine
Fees must be paid in CASH only ~ No checks, credit or debit cards will be accepted
Requirements:
*MUST bring all immunization records
*Parent or legal guardian MUST accompany their child ~ signature & ID is required
* No Insurance accepted * Be Prepared to wait.
